How do I make an appointment?
Appointments can be made by telephone. Telephone numbers are listed under Contact Us. Please have your insurance information available to give to the receptionist. If you are currently covered under a managed care plan, we can schedule your appointment more efficiently if you provide us with a physician referral number. (You will need to contact your primary care physician and ask for a referral in advance.) If for some reason the physician that you requested to see is not available, we will try to accommodate your request at the earliest time available or schedule you with another physician.
How do I obtain a refill of my prescriptions?
Please contact your pharmacy if you would like a refill on your prescription. Your pharmacy will send a refill request to our office. Allow 48 hours for our office to fulfill a prescription request.
What do I need to bring on my first visit?
Once you have made a new patient appointment, our office will send you a letter confirming your appointment, a Patient Brochure, and forms that we ask you to complete.
Please bring the following items with you to your appointment.
- Patient Information Form
- Health Questionnaire
- List of current medications OR bring your medications
- HMO authorization/referral if enrolled in a managed care plan
- X-rays, medical records, or any reports from your referring physician
- All insurance cards INCLUDING the Subscriber's social security number and date of birth.
- Write down any specific questions you would like to ask the doctor

Do I need to pay for my procedure/test prior to being seen?
Co-payments are expected at the time of check-in. Additional payments will be charged for related procedures and test. Urology Associates will bill your insurance carrier for the balance due.
Do you accept personal checks?
Yes, we accept personal checks.
Can I pay by credit card? Which ones do you accept?
We accept Visa, MasterCard, and Discover; as well as debit cards.
Do I have to pay my co-payment upfront?
Co-payments are expected when you check in for your appointment.
